Registration Process

Call Cummins Child Development Center between 6:30AM-6:00PM and inquire about general information of the center at 812.378.5833.
Download, print, and complete a registration form and pay the registration fee. The registration fee is non-refundable and is valid for one year from the date that you registered. The registration form and registration fee validates your child’s place on the waitlist; however it does not hold a spot. Cash or check is accepted. Checks can be payable to CCDC for the amount of $100.00. You may drop the fee off at the center or send it by mail to 650 Pleasant Grove 47201.
Once a spot is offered to you, you may download the enrollment packet from our website, complete it and return it to the center with your paperwork that needs to be completed by your child’s physician. After your enrollment packet is completed by a parent and a physician, please call the center and make an appointment to review paperwork for any outstanding forms due to the center prior to your child’s enrollment. Please know that if there are ANY outstanding immunizations that your child needs for his specific age you may not enroll until the immunizations have been administered. After you accept a spot in our center, you will work with a manager to schedule an appointment for a family intake meeting. This meeting is a mutual exchange of information and expectations; this is the parents’ chance to share in detail what they believe and what they wish to see for their child. This also includes a “No Surprises” discussion of real world issues that the parent may encounter – teacher turnover, biting, and other ups and downs.
